Plant is a Notocactus (so now Parodia) SP.

P.S. It's not really clear in the photo but interesting thing is if you look in the centre of the conjoined flower you can see a dark thing, closer inspection revealed to me that this is a partially formed style. so it has 2 1/2 thereof. crazy.

That's really, really odd. I don't suppose that you got a photo of the bud as it developed?

Has there been any damage to the plant or any signs of monstrose growth?

No mate! I didn't notice it until this afternoon. I literally did a double take on it lol, pardon the pun. double take Icon_smile

Well funny you should mention that because the growth point is A-OK i.e. no damage, but last season the growth point was an unsual form, I was hoping for a crest but no; it grew out normally. Here is a photo from last season. The flower is pretty out-there though ey.

Geez, that really looks like a crest was trying to develop. Bit of a shame that the plant grew out of it, really...

Yeah I know, I was a bit disappointed when it grew out normally, but oh well.
The flower is quite cool in its' own right IMO, and now I have a new avatar hehe. double take Icon_biggrin

Good one Lewis! I did have an Astrophytum with a double flower once. I guess it's like when an egg develops a double yolk or two bananas form inside the same skin. You get two for the price of one! (Well maybe not with bananas because they're sold by weight!) lol! double take Icon_smile

I have a nice specimen of Noto Ottonis and had a very similar thing last season. Perhaps this is a noto trait ?
